Stop and think! Check the list below for the things you always do when you see your health care provider.

  • I never forget to bring a written list of my medicines and their doses.
  • I tell my health care provider if I have pain in my belly.
  • I tell my health care provider if I have fevers or chills.
  • I tell my health care provider if I have black or tarry stool.
  • I keep track of my weight and tell my health care provider if I am getting too heavy or too skinny.
  • I mention if my belly seems bigger or my ankles are swollen.
  • I tell my health care provider if I am sleeping during the day, if I am confused, or if I can't focus or drive.
