Vitamin K is a co-factor in the synthesis of clotting factors by the liver. In liver failure, INR is prolonged because of an inability of the liver to synthesize clotting factors; the administration of vitamin K will not improve INR. Vitamin K administration will improve INR when the liver has a normal synthetic function but vitamin K is not being adequately absorbed from the gut, as occurs in cholestasis (bile is needed to absorb vitamin K).
