Primary sclerosing cholangitis can present as intrahepatic (the so-called "small duct PSC"), extrahepatic or both. When viral hepatitis leads to cholestasis (which it does in a small percentage of patients, particularly adults with hepatitis A), it is intrahepatic. Primary biliary cirrhosis and sarcoidosis are granulomatous liver diseases that affect only the intrahepatic ducts.
